Branch Representative Visit Form

This form has been introduced by the League Support Working Group, to collect information from local Branches for the larger Performance Measurement Framework to help the Leagues tell our story and better illustrate our contributions and roles to the cadet program to our DND partners. This form is to be completed by the Branch Rep, not the Support Committee. Input from our DND partners may be required on a few questions within the form.

Instructions:
